We welcome referrals directly from physicians and other providers! If you would like to make a referral for your patient/client, please review the information below:

• Providers complete and sign the referral form linked below, faxing it to (336) 370-5005. 

• When the referral is received, we will contact the patient/caregiver to schedule an appointment.

• Unless the patient has recently received a speech-language evaluation and results are available, we will conduct an initial evaluation. If therapeutic services are required, we will make treatment recommendations. 

• Currently, we are out-of-network with insurance companies and are self-pay only via cash, card, or check. We do accept HSA/FSA and can provide patients with more information about using Superbills for possible reimbursement through their out-of-network benefits. Please contact us with any additional questions or concerns!
